Patients may reach our scheduling department by calling (480) 558-7078 during our regular office hours.
We require 24 hour cancellation notice. If you do not cancel within 24 hours you will be charged a $20.00 fee.
To save time, patients can download the forms below and have them pre-filled when they get here.

RX Refill Procedure
If you need a prescription refilled please call your pharmacy. Remember when doing so you may need to see the doctor before we are able to refill your prescription, we will contact you if that is the case.” There is a 24 Hour Turn Around time for Refils so please leave enough time.

Payment Policy
Payment for all services is due upon receipt of services. If a patient does not have insurance please contact office for payment arrangement.
